How to Enter
Option 1 – Referral Entries
- For each new patient you refer who starts orthodontic treatment (Invisalign® or braces) with Integrity Orthodontics during the promotion period, you earn one (1) entry into the Smile Into 2026 E-Bike Giveaway.
- There is no limit to the number of referral entries you can earn.
Option 2 – Free Online Entry (No Purchase Necessary)
- To enter without a referral, treatment, or payment, complete the online entry form on this page during the promotion period.
- Limit one (1) free online entry per person.
Option 3 – Free In-Office Entry
- You may also fill out a paper entry form at Integrity Orthodontics during regular office hours.
- Limit one (1) free in-office entry per person.
Entries received through any of the above methods are combined into a single drawing.
The Prize
- One (1) winner will receive a brand-new e-bike (approximate retail value: $1,299).
- Prize is not redeemable for cash and is non-transferable.
- Winner is responsible for any taxes, registration, or ongoing maintenance associated with the prize.
Key Dates
- Promotion period: November 1, 2025 – January 31, 2025
- Winner selected: On or about February 10, 2026
Eligibility
- Open to legal residents of Georgia, age 18 or older.
- Minors may participate through a parent or legal guardian.
- Employees of Integrity Orthodontics, its affiliates, and their immediate family members are not eligible to win.
- Void where prohibited.
Winner Selection & Notification
- One (1) winner will be selected in a random drawing from all eligible entries received during the promotion period.
- The drawing will be conducted at Integrity Orthodontics.
- The winner will be notified by phone and/or email using the contact information provided at entry.
- If the winner does not respond within 7 days, an alternate winner may be selected.
Taxes & Reporting
- If the value of the prize is $600 or more, the winner will be required to complete an IRS Form W-9 before receiving the prize.
- A Form 1099-MISC may be issued for the value of the prize in the applicable tax year.
- Consult a tax professional regarding any tax obligations.
Publicity & Liability
By accepting the prize, the winner:
- Authorizes Integrity Orthodontics to use their name, likeness, and photo in marketing materials related to this promotion, without additional compensation, where permitted by law.
- Releases Integrity Orthodontics, its owners, and employees from any liability related to participation in the promotion or use of the prize, including but not limited to injury, loss, or damage.
